If no change is required, click on the SET Screen Button . This action opens the primary Ticket Format window shown in Figure 22 . 7 . The following four (4) Forms are displayed by the Ticket Format window:   Form - 1 : Is used for individual Machine Tickets . The top portion of the form is where the customer can enter specific information, such as Name, Address and Telephone Number . The Machine Code section is where each machine can be assigned a unique 14 - digit identification number . The number can reflect the location, asset number, et - cetera (Review Figure 22) .   Form - 2 : Is used to manually establish a sequential series of Machine Number Tickets . However, the customer identification information must be entered as on Form - 1 ( See Figure 23) .   With the power OFF, open the printer and turn DIP Switch #1 to ON ( See Figure 24) . This is for printing sequential tickets using the Ticket Format sub - window in Form - 2 and Form - 3 .   Form - 3 : Once an Excel (or other) database file has been converted to the . csv format, use the CSV file load button to download the file ( See Figure 25) .   Form - 4 : This form is not being used at this time . NOTE: The “Attr Bar Code” window must always show an 01 (See Figure 22 a) .
